Documents Needed
If your child has previously attended another school, please provide:
- Name, address, and phone number of existing school attended so we can request records
- Updated Immunization records
If this is your first time enrolling in any school provide the following documents:
- Birth Certificate
- Immunization Records
- Physical (A copy of a Well Child Check from a recent Dr's visit is acceptable for younger students)
